I have seen one reference that suggested that the Githyanki Lichqueen was dead. When and how did that happen? Was that a late 2nd edition module that I missed?

Nope, she was alive and kicking in 2e. The 'Incursion' module in Dragon magazine introduces the idea that she might have been killed since it's the goal of the PCs therein.
