I'm looking for D&D material that describes the Maiden of Pain as a character -- i.e., not her nature or domain (which are pretty well described), but any actual appearances of Loviatar in D&D literature (as an avatar, disembodied voice, etc.).

I figure she probably makes an appearance somewhere in the old Time of Troubles trilogy, but are there any other sources?

Loviatar came to D&D from the Finnish epic Kalevala. You can read a part with her in it here.

There is a book set in the forgotten realms which features a low-level priestess of Loviatar as its main character. I can't recommend it though; nothing particularly useful or specific about the goddess, not my favorite plot either.
