So, yes and no-- she didn't go around killing a githwarrior who failed at mending a boot or burned dinner. It wasn't even a necessary punishment for losing a minor skirmish. But yes, if a gith was entrusted with a responsibility of particular importance, then they'd better succeed.

Yes, I did see her as quite insane in my games. You know how Darth Vader killed people left and right in ESB? Well, I took that in my games and made it much more random.

No one questioned the Lich Queen when she just pretty much randomly stole life energy from even loyal subjects for a fit of pique.
