my initial Initiative was 21, and i "storywise" lowered it to 19 for posting later, now in my next post, in the 2nd part of the attack in the safe, do i get Ini 21 again or am i stuck at 19?, or do i roll again?

1 AoO per round, maximum. That is, unless you have combat reflexes feat, in which case 1 AoO per opponent, to a maximum of AoOs = dex mod per round. So still only 1 AoO in this case anyway.
Illithic head-grab works like thus (as per 3.0 rules, which I have) : Tenticles attack, the ones that hit are attached. The illithid can try to attach the rest with an opposed grapple next round (which does not itself provoke an AoO), if successful all 4 are attached. On its turn on the 3rd round, IF all 4 tenticles are still attached, the illithid rips out Ii's brain.
Well, since a tenticle does d4+1, Ii takes no actual damage. That is, the damage taken does not slow Ii down, not that the illithid can't get through her skin. I'd understand if you argued with me, but I think its a little extreme to say Ii can't be de-brained. Its just that she's so tough that the tenticle damage has no real effect on her combat-wise.
DR as listed has no effect on grapples. For that matter, all I'd have to do is rule Lavendar has a feat to increase tenticle damage, or apply a strenth mod, and Ii would possibly take some damage, which means anything accompanying the damage happens as well.
So: DM ruling is that Ii takes no HP damage from the tenticle attacks, but still has to worry about that 4 tenticle grapple.

What is Ii's AC? I think all the tenticles hit, however I will rule that the illithid still needs to win a grapple ( +2 per tenticle on) for a round before it can extract on the third round.
That means at least two rounds of full attacks from Ii. Ya'shenn still gets an action in the fumble/tenticle/AoO round first, though.
For your info, the Illithid's AC is 13 right now. Touch AC 10.

OK, here's the deal: Ii won the opposed grapple. This means the illithid DID NOT manage to attach the 4th tenticle, and cannot eat her brain just yet. I used the first grapple check for that.
Ii has a few options (choose one)
1) continue to attack the illithid (she would normally have to use a small weapon to do so, but close quarter fighting allows her to continue using the morningstar)
2) try and escape the 3 tenticles that are attached. This will require another opposed grapple roll (on Ii's turn). Since you already rolled multiple, I'll use the 2nd roll and roll for Lavendar. If Ii wins, she gets the illithid off her.
3) something else
EDIT: I can't find anything that says you get a bonus on the grapple check for the damage you did in the AoO, so Ii wouldn't have won the opposed grapple without that + 16 ( she only gets full attack with strenth bonus) if I had used the first roll. HOWEVER, I used the last roll, which DOES beat Lavendar's grapple, so still only 3 tenticles on (aren't I nice?)
Ii actually gets attacks before that opposed grapple (full attacks or escape attempts with full attack bonuses), as does Ya'shenn, if she wants to do anything.
I won't reveal HP totals, but a little info could help you make a decision. The illithid is severely wounded now, 25% or fewer HP.
Ii will get De-brained if Lavendar has 4 tenticles on at the beginning if its next turn. Before that happens, the following will occur:
1)Ii acts (this round)
2)Ya'shenn acts
3)Ii acts again (next round)
4)Ya'shenn acts again
5)Lavender tries to get tenticle 4 on again
6)Ii attacks againm
7)Ya'shenn acts again
8)Gy-Nath and Du'minh come running to the rescue. Big dip in Ii's personal honor if it comes to this, but they are free of their own combats by now and close enough to move-attack.
The feat Close Quarter Fighting gets me that ability, i have copied and send it to y'r email to clarify.

i need more attacks of opportunity to keep each tentacle from grabbing me (as in: i can make a close quarter attack for each tentacle, if i was allowed to), but you do have to make opposed grapple checks for each tentacle, not auto grapple just after 1 attack..

Illithid don't have to do a grapple for each tenticle. This might have changed in 3.5, but I have 3.0. Attack rolls vs AC, tenticles that hit are attached, then on later rounds only 1 opposed grapple check to attach "the rest of the tenticles"
However, I don't think it's fair to make Ii drop the morningstar, she can still full attack with it.
Still, only 1 grapple roll, so it doesn't matter if I take the first or last, you won, no more tenticles attached, but you'd have to initiate and win a grapple or Escape Artist check to get the illithid off you completely.
So, DM ruling is that the illithid hit with 3 tenticles, and failed to grapple-attach the 4th.
Just a comment, but I wouldn't mind if Ya'shenn (at some point soon) has a chance to do something about the large number of thralls that the party will probably be encountering within a few rounds. I'm not sure/don't recall how far each of the corridors outside the vaults extends, but as a ranged character, a round or two to toss a Negate Psionics (or AoE abilities/thrall-suppression toys) before they attack the vault might be nice.
Ya'shenn can't attack this illithid, and can't, at the same time, leave it alive, so I don't have much to do.

You can attempt to trip on an AoO, at least in 3.5.
I expect we'll be encountering more Tamers with 2h weapons and weapons with reach. (Spiked chain + Improved Trip maybe...)

The combat is in round... 5 or so? (illithid hides in vault, mind blasts, grapple, suggestion x2, probably at least round 5)... and Lavendar screamed to Violet for help in round 1 or 2. Assuming Violet is in range (likely, It is on the same floor at least) and opts to fight and not flee, Ya'shenn figures there will be lots of thralls and/or a second very angry illithid in the room very soon. The fact that taking out Lavendar is taking so long is making her very frustrated.

I think Lavendar's intent here is that, if Ii delays long enough to torture him, either Lavendar actually has a chance to survive the encounter, or at least that Violet will be able to defeat the githyanki.

You know, I've kind of wondered what illithid 'naughty bits' look like. (They're pure hermaphrodites, lay eggs, and normally have no urge to mate -- no need to produce hormones. They're probably a bit 'different' from the humanoid versions.)

well yes, i have been thinking along similar lines, but in regards to how it must really feel like to hack someone apart in combat, we nowadays have no concept of this kind of danger, combat being as far as it is done, a far away and unemotional thing, bullets with massive range and weapons that are much more efficient...
we dont "see" the person die on our feet on average anyway...
I am not a pacifist (being a bouncer), but splitting someone in half is something altogether different...
I'm not the DM, but Daraz is not a telepath, so I doubt he 'heard' Lavendar trying to contact Violet. Ya'shenn, however, has informed the others that Violet is (most likely) on the way.
Note, tho, if I were Violet, I'd send some thralls into the room first.
