Has anyone actually rendered up a planescape version of these.
if so, where can they be found, i have ran across many 2nd ed, but no 3rd, 3.5 or 4th editio versions.

Well I don't know how much different a sheet would need to be for a PS campaign. But I added a line for Affiliation/Rank/Score to keep track of Faction affiliation info. Other than writing your faction on the sheet, I don't know what else would be planar specific for a character sheet.
