I have heard a rumor that some First Edition book contained a Mind Blast results table with many interesting results other than stunning -- insanity, for example, among others. Does anyone recall which book contained this table?

That was in the 1e DM guide. It was based on the attacker's and defender's psionic strenths. There was another section in the DMG that dealt with various forms of insanity.
